Our providers help patients with prevention and wellness strategies, and will coordinate all appropriate health screening tests. As a primary care provider, our physicians and allied staff will also coordinate any care their patients may need from other Premier Medical Group or other specialists.nnThe Physician Assistant/ Nurse Practitioner is responsible for assisting physicians as well as seeing patients independently for office visits.nnESSENTIAL DUTIES AND RESPONSIBILITIESnnWorks in collaboration with physicians and other health care providers to provide health care across the continuum of care to assigned patients.nnPerforms hospital rounds of new consults and follow-up patients and assists in discharge of patients.nnObtains patient medical, physical and psycho-social history and performs comprehensive health assessment including physical exam, based on age and history.nnConducts non-invasive preventive screening procedures based on age and history.nnIdentifies medical and health risks and needs. Updates and records changes in health status in EMR.nnFormulates the appropriate differential diagnosis based on history, physical examination and clinical findings.nnOrders and interprets appropriate diagnostic tests and laboratory studies within protocol guidelines.nnDevelops and provides a relevant patient education plan.nnPrescribes and administers appropriate pharmacologic and non-pharmacologic therapies.nnMakes appropriate referrals to other health professionals and community agencies.nnDetermines the effectiveness of the plan of care through documentation of client care outcomes.nnReassesses and modifies the treatment plan as necessary to achieve medical and health goalsnnCompletes all documentation in EMR in a timely fashion. Codes patient visits accurately and appropriately.nnParticipates in quality assurance review on a periodic basis, including systematic review of records and treatment plans.nnTreats patients and their families with respect and dignity.nnConsults other departments, as needed and as appropriate, to provide for an interdisciplinary approach to the patient’s needs.nnEDUCATION & EXPERIENCEnnGraduate of an accredited Physician Assistant/ Nurse Practitioner Program; NYS licensednn1 – 5 years related experience; or equivalent combination of education and experience.nnQUALIFICATIONS & REQUIREMENTSnnDemonstrates an ability to utilize advanced health assessment and decision making skills.nnUtilizes initiative; strives to maintain steady level of productivity; a self-starter; compatible and supportive; a team player.nnMaintains a good working relationship and effective communication both within the department and with other departments for the benefit of the patient.nnDemonstrates an ability to be flexible, organized and function under stressful situations.nnMaintains patient confidentiality at all times.nnRepresents the organization in a positive and professional manner.nnAttends outside clinical updates as required, and attends annual review and department in-services as scheduled.nnActively participates in performance improvement and continuous quality improvement activities.nnPremier Medical Group is an Equal Opportunity Employer
