This is a role that is key for service delivery within our Personal Care area and designed to truly make a difference in the lives of people that are served, improving the quality of life and connection to community.
Key Responsibilities
- Patient Intake and Assessment:
- Conduct comprehensive initial assessments of new patients, including physical, emotional, and environmental evaluations.
- Develop individualized care plans based on assessment findings, patient needs, and preferences.
- Collaborate with patients, families, and healthcare providers to gather necessary medical history and current health status.
Care Coordination:
- Coordinate with other healthcare professionals and agencies to ensure seamless patient care transitions and continuity of care.
- Schedule follow-up visits and ensure timely delivery of services as outlined in the care plan.
- Maintain detailed and accurate patient records in compliance with state regulations and company policies.
Regulatory Compliance:
- Ensure all patient assessments and care plans comply with North Carolina state regulations and guidelines for personal care services.
- Stay informed about changes in healthcare regulations and update practices accordingly.
- Participate in audits and inspections as required by regulatory bodies.
Patient Education and Support:
- Provide education and support to patients and their families regarding health conditions, medications, and care plans.
- Address any questions or concerns patients or their families may have about their care.
- Advocate for patients' rights and needs within the healthcare system.
Quality Improvement:
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Collect and analyze data related to patient care and use findings to implement improvements.
- Attend and contribute to team meetings and training sessions.
Qualifications
- Education and Licensure:
- Current RN licensure in the state of North Carolina.
