This position is located in our brand-new Oakland City Center location. This role is a hybrid opportunity that includes traveling to patient's homes for in-home visits as needed in the Bay Area.
About Your Work: The Sr. Manager will lead a team of Nurse Practitioners responsible for assessing and evaluate the health care needs of patients in Altais Health Solutions programs, including the Care Where You Are program, Annual Health Assessments, Clinical Chart Summary, supporting Altais Medical Group visits and operations, and other programs. This will be a working manager role whereby the Sr. Manager may also provide the services, including in-home needs evaluations for patients.
Lead and mentor a team of Nurse Practitioners, providing guidance, support, and training as needed.
- Collaborate with cross-functional teams, to ensure alignment with organizational goals for Care Where You Are and AHA.
- Review team performance metrics to ensure alignment with program commitments.
Performs Comprehensive Health Assessments of patients in person and via telehealth.
- Uses patient centered care concepts to manage acute and chronic conditions, as well as preventative care, within the full scope of clinical practice.
- Actively and accurately documents clinical information.
- Utilizes excellent communication skills to ensure continuity of care for the patient, which includes regular communication and co-management of care with Primary Care Provider, Specialists and Allied Health Providers.
- Identifies actual or potential health risks based upon observation of physiological and/or behavioral data and responds appropriately.
- Formulates concise problem lists based on clinical findings.
- Formulates a complete, concise, and clear plan including follow up care.
- Provides and evaluates patient care according to protocols and quality assurance program criteria.
- Submits and follows up on timely referrals for needed preventative care including labs and radiology, and addresses all aspects of a comprehensive evaluation including, but not limited to, Medication Review, Reconciliation Post Discharge, Transition of Care, Care for Older Adults, Meaningful use, Behavioral Health, Social Determinants of Health, Advance Care Planning, and Patient Experience
- Captures HEDIS Quality Gaps during appointments including Diabetes, Cancer Screenings, Nephropathy, and Statins
- Collaborates with HCC Coders on clinical documentation review before submission of chart for claim and diagnoses accuracy.
- Maintains a general knowledge of diagnoses codes and supporting documentation required.
Ensures best practice use of Electronic Medical Records and other tools provided:
- Performs certain functions and procedures, which overlap those of a physician; details listed out on the Family Nurse Practitioner Standardized Procedures in accordance to The Nurse Practice Act In The California Business and Professions Code, Chapter6 Article2, Section2725, and The California Administrative Code, Title16, Article7, Section 1470-4 and Title22, Division3, Section 51240(Medi-Cal).
- NP role is integral to the population health/proactive outreach activities of each team. As such NP participation in Interdisciplinary Team and Population Health Meetings and collaboration with team members including Social Work and Community Health Advocates is expected.
- Practice of evidence-based medicine and adherence to both national and internal practice guidelines and policies.
- Utilizes screening, assessment and treatment data to provide patient/family education.
- Will be required to attend documentation training, EMR Training, and telehealth portal training.

The Skills, Experience & Education You Bring - Active and good standing RN licensure and Nurse Practitioner Certification in the State of California
- Active and good standing NPI number
- 10 or more years related experience including 4 years of leadership experience.
- Post-graduate training in area of responsibility.
- 4+ years as a Clinical/Family Nurse Practitioner
- Possession of a current Furnishing Number issued by the California Board of Registered Nursing AND Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) OR Certification by the National Certification Corporation as a Women's Health Care Nurse Practitioner (WHNP).
