Performs and/or coordinates the assessment, planning, implementation, education and evaluation of the patient care provided in the home setting. Responsible for performance within the scope of the Nurse Practice Act of North Carolina. Performance is consistent with hospital and home health policies and State and Federal Guidelines. Is competent to deliver care to clients in Home Health as evidenced by the completion of the RN skills competency checklist. Graduate from an approved school of nursing. BSN in Nursing preferred and/or Diploma in Nursing Required. Current RN licensure in the State of North Carolina and valid NC Driver's License. Current BCLS certification required; specialty certification preferred. At least one year home health case management experience required.
