Behavioral Health (Greenhope Services for Women)nnLicensed Practical Nurse (Manhattan) New York, NY : 7/5/2024nnJob DescriptionnnJob ID#:nn3954nnJob Category:nnBehavioral Health (Greenhope Services for Women)nnPosition Type:nnPart-TimennDetails:nnThe part-timeLPNwill provide medical care and services for women with a history of chemical dependence, including health history, physical examination, minor office procedures, treatment, referral, counseling, and follow-up care and services at Greenhope Services for women and children.nnThe hourly rate for this role is $24.00/hr - $39.00/hr.nnRESPONSIBILITIES:nnUnder the supervision of a physician act as the health coordinator for outpatient and residential programs to provide services to women and children.nnReview referred potential clients' medical histories submitted by the intake department.nnAssist with referring clients to appropriate healthcare provider specialists.nnAssist with managing client prescriptions with various pharmacies, focusing on ChemRx.nnCoordinates with the client's primary care physicians to inform decisions regarding the management and treatment of problems identified and provides care as prescribed by the physician.nnManage client care: follow up plan and monitor progress, determine when referral to another provider is necessary and make arrangements for further care, determine when emergency room visit is necessary and when illness is resolved.nnWork with interdisciplinary team members regarding modification of treatment as necessary according to health concerns.nnProvide oversight and monitoring of the wellness center to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.nnDocument all medications in the electronic health record system, AWARDS.nnMay participate in staff meetings or professional development activities as needed.nnEDUCATION AND REQUIREMENTS:nnLicensed Practical Nurse (LPN) or Registered Nurse from an accredited Nursing school with appropriate educational degrees.nnState of New York license and certificationnn3+ years practical experience in health care facilitynnKnowledge of substance/alcohol dependence/abuse populationnnSpecialty areas may include but not limited to Family Health, Women's Health, Holistic Care, etc.nnBLS/CPR certification is requirednnAcacia Network is an equal-opportunity employer *nnJob Requirements
