Hiring Per Diem Hospice LPN's - Licensed Practical Nurses for the Greenwich, Stamford, Darien & Norwalk areas.
We are looking for LPNs with hospice and/or home care experience with weekend and weekday availability!
Job Summary:
The Licensed Practical Nurse (LPN) is responsible for providing end-of-life care, hospice and/or home care services to patients in their home or community setting. The Licensed Practical Nurse (LPN) is responsible for assisting in the planning for the patient's and family care under the direction of the RN and the supervision of a Registered Nurse. Responsibilities include following the plan of care established by the physician, providing treatments, and working collaboratively with the other members of the team to help meet positive patient care outcomes. The Licensed Practical Nurse (LPN) supports the philosophy, goals and culture of Constellation Health Services.
Requirements:
- Graduate of an accredited practical nurse program.
- Connecticut State Licensed LPN with a minimum of 1 year of clinical experience.
- Community home health care/hospice experience is preferred.
- Clinical auditing background preferred.
- Experience conducting chart reviews preferred.
- Wound care experience preferred.
Responsibilities:
- Provides nursing care as defined in State Nurse Practice Act, including specialized duties related to end of life care and/or home care.
- Implements current nursing practice following a comprehensive assessment and the plan of care.
- Maintains accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Recognize and interpret symptoms and institute remedial measures within the scope and limits of a Licensed Practical Nurse.
- Observes records and reports to immediate supervisor if any changes in patient's condition.
- Participates in coordination of hospice and other home care services, appropriately reporting the identified needs to the interdisciplinary group. Examples include RN, hospice aide, MSW, OT, PT, ST, Registered Dietician, Spiritual Counselor or Clinical Supervisor.
- Participates in IDT meetings
- Implements care to achieve outcomes of comfortable and safe dying and effective grieving for the patient and family.
- Uses and prepares equipment and supplies effectively, adhering to aseptic technique.
- Provides appropriate pain/symptom management.
- Evaluates the patient's response to treatments/medications.
- Performs other duties as assigned by the registered nurse or clinical supervisor.
