Description Position Summary Under the direct supervision of the Director of Nursing and/or Assistant Director of Nursing, the LPN coordinates and supervises the implementation of the nursing care plan for assigned patients of a geriatric patient population. Assists the registered nurse and acts as a charge nurse in the comfort, care, and treatment of patients. Performs a variety of semi-skilled nursing responsibilities, including administering medications.
Key Responsibilities - Provides patient care by:
- Assisting in assessing patient care needs. Observing and reporting changes to the Registered Nurse.
- Assisting with the planning of patient care utilizing Nursing Care Plans.
- Administering care as it applies to activities of daily living and recording in clear, concise nursing notes.
- Administering therapeutic care and recording in clear, concise nursing notes.
- Contributing to the formation of short- and long-term goals (patient and family teaching and discharge plans).
- Evaluating patient responses.

. Provides medications by: - Accurately delivering medications to patients observing the five rights of this process
- Observing Department of Nursing Policies and Procedures.

. Implements patient care by: - Administering care for activities of daily living.
- Administering therapeutic care to include admissions and discharges.
- Assisting with physical exams and diagnostic procedures as assigned.
- Collecting specimens.
- Providing postmortem care.
- Delivering treatments as assigned such as enemas, rectal tubes, postural drainage, dressing changes, and irrigation.
- Assisting with nourishments such as meals, supplements, and tube feeding.
- Applying isolation techniques and infection control procedures.
- Initiating CPR and assisting in emergency cart use.
- Making clear, concise nursing notes that reflect implementation of and patient response to the nursing care plan.

. Assists in evaluating patient care by: - Assisting in revising and updating nursing care and treatment plans.
- Participating in patient care and team conferences.

. Teaches and learns by: - Assisting other members of the health care team as requested.
- Participating in on-the-job training of personnel.
- Participating in teaching and reinforcing teaching plan to patient and family.

Participates in Leadership and Coordination by: - Assisting in establishing priorities in patient care.
- Assigning work to CNA's.
- Working as the charge nurse in the absence of the Director of Nursing or the Assistant Director of Nursing, providing general oversight and supervision for all staff assigned to department.
- Assisting the Director of Nursing and/or the Assistant Director of Nursing with staffing and scheduling needs of the department.

. Manages personal development by: - Participating in activities on unit to improve patient care such as QA, committees, etc.
- Initiating self-improvement.
- Attending six hours of in-services and continuing education programs.
- Participating in at least 75% of staff meetings.

- Demonstrating appropriate job-related behavior and following Standards of Conduct.
- Annually attending required courses.
- Complying with annual employee health assessment.
- Complying with bloodborne pathogen procedures.
- Complying with body substance isolation procedures.
Qualifications Minimum Qualifications - A high school diploma or equivalent.
- Licensed as a Practical Nurse in the state of Georgia.
- Currently certified in Basic Cardiac Life Support (BCLS).
- Two years' experience as LPN.
- Ability to do frequent lifting and turning of patients, singularly and with other staff members and equipment; and ability to interact professionally with patients and families.
- Ability to stand or walk 90% of the day; Ability to bend and stoop frequently.
- Near visual acuity needed to work with charts. Hearing and speech are essential to communicate with patients and residents.
