Schedule/Shift: 4x10s can be worked any days of the week based on your preference and availability; Monday - Sunday availability; either 8:00 AM - 6:30 PM or 10:00 AM - 8:30 PM
Daily Visits: Typically 5-8 in-home patient visits typical time between patients 30-45mins
Job Summary: We are seeking a Licensed Practical Nurse (LPN) to join a growing healthcare team in NYC. The Licensed Practical Nurse (LPN) will provide in-home patient care services with a focus on underserved populations as part of a Healthcare Plan Partnership Program. Clinical duties include point-of-care tests, health screenings, wound care, and more. Driver provided to those who do not have an active NY Driver License. LPNs will either drive the company vehicle to patients homes or be driven by a company driver.
Responsibilities:

• Perform point-of-care tests (Blood Glucose Testing, nasal swabs, Urinalysis, etc.)
• Administer EKGs
• Assist with proctored eye exams
• Conduct blood draws for lab send-outs (IV/Blood Draw certification required)
• Provide non-complex wound care and vital signs monitoring
• Facilitate video visits between patients and healthcare providers
• Perform health screenings and administer fluoride varnish treatments
• Ensure accurate documentation of patient visits
• Work closely with underserved communities, providing compassionate and efficient care in patients' homes

Minimum Skills/Qualifications:

• Licensed Practical Nurse (LPN) in the state of New York
• Comfortability with in home care, phlebotomy and IV placement required
• Either a NY ID or an Active NY Driver's License
• Ability to lift 40 pounds and handle mobile medical equipment
• Bilingual in English and Spanish (preferred)
• Familiarity with underserved communities and providing care in home environments (preferred)
