If you are passionate about making a difference in the lives of seniors and thrive in a collaborative setting, we encourage you to apply Position Summary: The Occupational Therapist (OT) is responsible for assessing, planning, and implementing individualized therapy plans to enhance the functional independence of residents. This role includes working with patients, families, and the care team to achieve measurable goals that improve residents’ quality of life. Key Responsibilities: Conduct comprehensive assessments of residents’ physical, cognitive, and psychosocial abilities. Develop and implement personalized treatment plans focused on restoring or maintaining residents' independence and safety in daily activities. Provide therapeutic interventions that address motor skills, coordination, and adaptive techniques. Collaborate with an interdisciplinary team, including physicians, nurses, physical therapists, and speech therapists, to ensure cohesive care plans. Educate residents and families about therapeutic goals, techniques, and adaptive equipment. Maintain accurate and timely documentation of evaluations, progress notes, and discharge summaries in compliance with state and federal regulations. Monitor residents’ progress and adjust therapy plans as needed. Participate in team meetings and contribute to ongoing quality improvement initiatives. Qualifications: Bachelor’s, Master’s, or Doctorate in Occupational Therapy from an accredited program. Current and valid Occupational Therapist license in the state of Colorado. Experience in a skilled nursing or rehabilitation setting preferred. Strong interpersonal and communication skills with a team-oriented approach. Proficiency in documentation and use of electronic medical records (EMRs). Dedication to providing high-quality care with compassion and respect for residents' dignity and independence.
