As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:
n
n- Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
n- Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
n- Take patient vitals, do assessments, document progress and report changes to RN supervisor
n- Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
n- Educate family on the patient’s plan of care, medication and home safety
n- Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
n

A few must-haves for Home Health Licensed Practical Nurses:

n- Graduate of an accredited practical nursing program and active LPN license in NJ
n- Knowledge of state and federal home health regulations
n- Clinical proficiency in nursing, strong communication skills and compassion
