Job Description

The Long Term Care LPN assumes responsibility and accountability for the delivery of care for patients in a Long Term Care/Assisted Living Facility. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance ability in the coordination of patient care. Additionally, this individual performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.nn10p-6annOrnn2p-10p/10p-6p (mix between the two shifts notdoubles)nnEssential Duties and Responsibilities:nnUtilizes the nursing process to assess, plan, implement and evaluate patient care in a Long Term Care/Assisted Living FacilitynnPerforms focused interview to identify specific needsnnAssess signs and symptoms indicating physiologic and psychological changes in the patient’s conditionnnCollects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnosesnnDocuments the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventionsnnIdentifies and prioritizes actions based upon patient care requirementsnnMonitors consistency between patient plan of care and actions of medical staffnnPerforms interventions according to identified priorities, plan of care, and the facility policies and patient outcome standardnnProvides a safe, comfortable and therapeutic patient environmentnnInitiates and evaluates patient and/or family educationnnRevises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventionsnnDocument’s patient’s response to nursing and other interventionsnnUses clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing prioritiesnnMonitors safety and effectiveness of the environment and equipmentnnParticipates in activities to facilitate good interpersonal communications with the chosen facilitynnSupports professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient’s Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policiesnnPerforms other duties as assigned/necessarynnMinimum Requirements:nnCurrent Licensed Practical Nurse License for the state in which the nurse practicesnnAbility to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skillsnnCurrent Health Certificate (if applicable)nnCurrent PPD or Chest X-RaynnOne year prior professional nursing experience preferrednnCurrent BLS cardnnSuccessful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greaternnSuccessful completion of Background Screening ProcessnnBenefitsnnAt Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits:nnCompetitive pay & weekly paychecksnnHealth, dental, vision, and life insurancenn401(k) savings plannnAwards and recognition programsnn*Benefit eligibility is dependent on employment status.nnAbout AmergisnnAmergis, formerly known as Maxim Healthcare Staffing, has served our clients and communities by connecting people to the work that matters since 1988.
