Job Summary
The Anticoagulation RN is a register professional nurse who, in collaboration with the medical Leader, manages patients who have anticoagulation management requirements involving administration and monitoring of Warfarin (Coumadin). The Anticoagulation Clinic provides education to enrollees, and ongoing monitoring of PT/INR (prothrombin time/international normalized ratio), with continual recommendations for warfarin dose adjustment including drug-drug interactions and sub and supratherapeutic INR.
Job Specifications
EDUCATION AND EXPERIENCE:
Currently licensed RN in the state of Illinois. Certification and maintenance in or progression toward certification in anticoagulation therapy. Competent in CPR. Prefer minimum of 2 years or more experience in medical/surgical nursing.
JOB KNOWLEDGE:
Adult teaching learning principles
Lab tests in general but in-depth knowledge of INR and PT
Anticoagulation medications
Interactions of food and anticoagulation medication
Diseases requiring anticoagulation medications to include but not limited to deep vein thrombosis and/or pulmonary embolism, atrial fibrillation, mechanical heart valve replacement, and many other cardiovascular or circulatory diseases

The RN will successfully complete orientation and hospital-wide and department-specific CBL's (computerized based learning). The RN will participate in departmental inservices and will demonstrate competency by completing the service specific competency testing / evaluation process, as appropriate to the RN role. Continuing education programs will be offered to augment the RN's knowledge of pertinent developments in patient care and to foster a performance improvement environment.
Must have the ability to work with a variety of patients of all populations (e.g. developmentally appropriate care, cultural diversity, patients with pain, victims of abuse or neglect, and diagnosis or procedure specific such as patient with congestive heart failure or cesarean delivery).
