This position is located in our Remarkable Hospice Dublin location with a sign-on bonus of $500 .
n
Duties include:
n
n- Assessing and providing hands on care
n- Admissions after hours
n- After hours discharges including death pronouncement
n- Communicating with the interdisciplinary team
n- Documenting necessary information in the electronic healthcare record
n- Collaborating with the hospice physician
n- Coordinating necessary ancillary services such as DME, lab, xray, transportation
n- Providing services collaboratively with other healthcare providers such as in nursing facilities, hospitals and home health
n- Oversight and direction of unlicensed assistive personnel
n- Education and support for family, caregivers and facility staff
n

nRequirements** :
n
n- Position requires dependable transportation without restriction.
n
n
Other requirements include:
n
n- Unencumbered licensure.
n- Current healthcare provider CPR
n- Ability to perform essential functions of job with or without reasonable accommodation
n- Must pass criminal history check and other required background checks
n- Drug and alcohol screening
n- One year of registered nursing experience. Two years with hospice preferred
n- Bilingual English and Spanish or other languages a plus
n- Homecare Homebase experience a plus
