- This is a full-time, Monday through Friday, Daytime position.
- This position requires a valid Pennsylvania Licensed Practical Nurse license, a valid Pennsylvania driver's license, and reliable internet access.

As Homeland Hospice's Licensed Practical Nurse (LPN) you would provide skilled nursing care to the hospice patient, and support to the family and caregiver, as directed by the supervising Assistant Director of Clinical Services, in compliance with Medicare Hospice Conditions of Participation, Pennsylvania licensure laws and agency policies and procedures. Provide timely documentation of patient services based on the patient's plan of care. Works closely with other members of the Interdisciplinary Team (IDT) to ensure all patient and family care needs are being met.
Requirements include: - Education and Experience
- Graduate of accredited practical nursing program.
- Minimum age requirement of 18.
- Has minimum of one (1) year general nursing experience. Community health/hospice or medical/surgical experience is preferred.
- Ability to work within an interdisciplinary setting.
- Understanding of Hospice philosophy and needs of terminally ill.
- Licensure, Registration and/or Certification
- LPN licensed in the Commonwealth of Pennsylvania.
- Valid driver's license and reliable transportation that is insured in accordance with Pennsylvania state requirements.
- Satisfactory references from employers and/or professional peers.
- Satisfactory criminal background check.
- Maintains current CPR certification.
- Must have reliable internet access.
