Title: Licensed Practical Nurse (LPN)
Reports to: Nurse Manager
FLSA Status: Non-Exempt
Personnel Supervised: None
POSITION SUMMARY:
The Licensed Practical Nurse is responsible for providing direct and indirect nursing care to patients and families in the clinic under the direction and supervision of the Nurse Manager and Director of Nursing (DON). The LPN is expected to work well with every member of the clinical team to provide safe, high quality and efficient primary care to a patient population diverse in age, health status, and socio-economic level. This position requires strong team work and communication skills with the care team.
MINIMAL QUALIFICATIONS:
  • Education: Graduate of an accredited School of Nursing
  • Licensure: Florida license
  • BLS certification
  • Computer literacy
  • Experience in Community Health Preferred

RESPONSIBILTIES AND PERFORMANCE EXPECTATIONS include, but are not limited to, the following:
  1. Be familiar with CFHC's program philosophies, goals and objectives. Be cognizant of and comply with all CFHC's policies and procedures, as well as state and federal regulations.
  2. Be competent in performing the duties of a nurse.
  3. Understand the requirements of CFHC's Exposure Control Plan. Personally, utilize universal precautions when practicing in a clinical setting.
  4. Participate in staff outreach and off-site health care programs as requested by the DON, Chief Medical Officer (CMO), represent CFHC in the community.
  5. Report as needed to DON any pertinent information or situations that impact on patient care or CFHC liability.
  6. Attend and participate in mandatory CFHC meetings (Center Specific meetings, Corporate meetings, and other meetings as designated by the DON or CMO).
  7. Demonstrate ability to work cooperatively with other members of the clinical team. Be supportive of coworkers.
  8. Maintain a neat and professional appearance at all times.
  9. Carry out any other activities deemed appropriate by the DON or CMO.
  10. Screen patients according to CFHC's protocols (example, obtaining height, weight, vital signs, vision/hearing testing) and perform any other procedures directed by the provider, Nurse Manager, or DON.
  11. Accurately obtain patient's medical history.
  12. Perform accurate charting in patient's medical record in accordance with CFHC's policies and procedures.
  13. Perform waived lab analysis (ex: urinalysis, pregnancy test, hemoglobin, glucose, strep, flu)
  14. Administer injections according to position requirements (example, subcutaneous, intramuscular, and intradermal injections).
  15. Prepare and administer vaccines/medications as directed by provider.
  16. Perform procedures, as ordered (ex: nebulizer treatments, ear lavage).
  17. Counsel patients per treatment plan (ex: diet, exercise, medication).
  18. Understand all emergency protocols and respond accordingly.
  19. Maintain exam rooms and work areas (ex: keeping rooms well-stocked, orderly and clean, to include disinfecting exam tables between patients as per infection control policy).
  20. Maintain strict infection control (ex: sterilizing instruments, performing procedures, needle disposal).
  21. Accurately make/cancel/reschedule appointments in the computer appointment system. Follow-up on appointment to determine patient compliance and contact patients to reschedule (no-show policy).
  22. Understand and assist in the referral process of patients to outside services or providers, follow-up on patient referrals.
  23. Complete and ensure accuracy of clinical logs (ex: hospital, temperature, medication/vaccine administration).
  24. Maintain patients concerns within electronic medical records (EHR), communicate with patients via telephone, the patient portal, and other modalities; keep providers and other team members updated regarding clinical changes.
  25. Complete all duties, responsibilities, and competencies in a manner that is appropriate to the age groups to whom care/services are being provided.
  26. Other duties as assigned by the clinician, Nurse Manager, DON, or CMO.
PHYSICAL REQUIREMENTS: (Subject to Change)
  1. Requires 80% or more time spent standing/walking
  2. Independently mobile
  3. Ability to lift weight equivalents to what would be required when occasionally assisting and positioning patients, repositioning equipment and lifting supplies
  4. Ability to adapt and function in varying environments of workload, patient acuity, worksites, and work shifts.
