The Nurse Practitioner will provide comprehensive patient care in the home working as the clinical team leader for our home health, palliative care and hospice teams.

Responsibilities
• Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
• Manage acute and chronic conditions, including medication adjustments.
• Order and interpret diagnostic tests.
• Develop, implement, and modify treatment plans as needed.
• Participate in quality assurance and improvement activities.
• Provide clinical education and training for patients, families, and facility staff.
• Adhere to state-specific NP scope of practice requirements.
• Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.
Qualifications
• Master's degree in Nursing or RN license.
• Advanced Practice Nursing license.
• NP Board Certification from recognized certifying body.
• Credentialed with insurance companies preferred.
• Fingerprint clearance.
• Unrestricted prescribing and dispensing privileges with DEA licensure required.
• Driving is a requirement for this position. Current state driver's license and proof of insurance is required.
• 1 to 2 years' NP experience preferred
• Experience with adult, geriatric populations and population health management preferred
• Strong communication, organizational, and customer service skills
• Knowledge of Skilled Nursing/Transition Rehabilitation Centers (preferred)
• Ability to handle a high volume of patients with skill and confidence
• Knowledge and use of an Electronic Health Record
• Ability to work with confidential information including HIPAA protected information
• Ability to be proactive and initiate processes with little to no supervision
