NURSE PRACTITIONER/PHYSICIAN ASSISTANT
Full Time, Days
Job Description:
- Provides client services to local Skilled Nursing & Rehabilitation facilities.
- Assures quality nursing care for patients by directing and coordinating nursing care.
- Conducts physicals and annual assessments for assigned patients.
- Completes acute visits to assess and diagnose patient conditions.
- Focuses on early identification of changes in condition as well as strong management of chronic conditions.
- Completes history and physical on new admissions within 72 hours of admission or readmission.
- Completes Medicare certifications according to required time frame.
- Prepares discharge orders, instructions and prescriptions for patients being discharged.
- Encourages advanced care planning and end of life decisions for patients requiring these services.
- Provides pharmacy with needed narcotic prescription refills as patient need dictates.
- Documents patient assessment and plan of care in the electronic medical record.
- Reviews labs and other diagnostic tests for additional interventions.
- Collaborates with physicians and staff.
- Notifies family of changes in condition.
- Communicates daily with nursing staff, assigns duties, supervises, and evaluates work performance.
- Reviews nursing policies and procedures for compliance, and to make recommendations for changes/additions as necessary.
- Recommends or conducts specific areas of in-service or continuing education.
- Assesses patients for palliative care needs and creates/oversees the palliative plan of care In facilities and in patient's homes.
Job Requirements:
- Current, unrestricted valid Nurse Practitioner license from the Board of Nursing/PA License from the Medical Board in the state of assignment or the ability to obtain.
- DEA certificate current or the ability to obtain.
- Clinical background in adult, family or geriatric specialties preferred.
- Past experience working in long-term care, a nursing home, or with seniors in other settings preferred.
- Strong communication and relationship building skills preferred.
- Intermediate level of proficiency in PC based word processing, including Microsoft Word and Outlook.
- Willing to be 'on call' for Facilities as needed.
- Dependable, flexible, and able to work and cooperate well with all nursing personnel and other departments.
- Ability to make decisions regarding nursing problems, work well under pressure, and problem solve.
