- Board Certified
- State License - ND License is preferred - IMLC is okay
- Residency Requirements/Fellowship - REQUIRED
- ACLS - REQUIRED
- Malpractice - Clean Preferred
- Availability at time of name clear - REQUIRED
- Must have experience with all required procedures
