Job Summary Serves as a member of the healthcare team to apply the clinical processes to make nursing judgments and decisions specific to assessment, care planning, evaluation, implementation and delivery of patient care. Documents relevant data and care processes appropriately. Works closely with patients using health coaching techniques to assist them in self-management of their illness(es). Facilitates communication between the patients and their primary care providers. Responsible for history taking and preparation of patients for examination as well as assisting with exam or procedure as requested by the provider. Performs duties consistent with the needs of the communities served, in a manner which reflects the physical, psychological and spiritual healing ministry of Christ. Qualifications - Education :
- ASN - Must have successfully completed an accredited registered nurse training program. BSN preferred.
- License :
- Current Indiana licensure and CPR is required.

- One to two years of experience in an outpatient setting preferred.

Performance Requirements - Provide clinical leadership with hands-on care to patient as practice transforms to value-based care model.
- Is aware of scope of practice boundaries and practices to top of license (e.g. co-visit with providers, billable nurse visits, etc.); is comfortable seeking direction and assistance from appropriate resources.
- Exercises professional nursing judgment when performing a broad range of procedures and treatments appropriate to the level of care needed, and prioritizes patient acuity.
- Conducts a patient-centered assessment utilizing applicable ambulatory nursing protocols and techniques.
- Synthesizes available data, information and nursing knowledge relevant to the presenting health situation to identify unique medical / nursing needs and implements appropriate interventions for age specific categories.
- Maintains and/or orders appropriate stock in exam rooms and medical supply inventories.
- Schedules procedures and referrals with adequate considerations for insurance pre-certification requirements.
- Administers injections, medications, immunizations, dresses wounds/incisions; and communicates provider instructions to patients, including vaccine information forms.
- Responsible for sample medication oversight and the proper recording and distribution of such medication.
- Performs point-of-care testing for patients and maintains compliance with testing requirements.
- Performs phlebotomy for patients and follows laboratory protocols.
- Works with clinical team and practice manager to delegate the following:
- Cleaning of treatment rooms between patients and at the end of the day. Documents on cleaning logs.
- Conducts proper cleaning and storage of all equipment used in patient treatment.
- Disposes of all toxic waste in accordance with local, state, and federal guidelines.
Physical Demands : - Requires fully range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time. Requires occasional lifting and carrying of items weighing up to 50 lbs and 100 lbs with assistance. Requires corrected vision and hearing to normal range. Requires working under stressful conditions and must be able to work irregular hours. Requires exposure to communicable diseases, toxic chemicals and/or fumes, and potentially dangerous body fluids.
