Our CompanynnAdoration Home Health and HospicennOverviewnnPrivate Duty LPN – Full-Time ShiftsnnLocation:Rhea County – Dayton, TN (37321)nnOverview:Adoration Home Health & Hospice is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide one-on-one care for a 9-year-old male patient. This role involves providing 24-hour care in 12-hour shifts, focusing on ventilator and tracheostomy management, G-Tube care, and supporting a patient with Down syndrome.nnSchedule:nn12-hour shifts: 7:00 AM – 7:00 PM (24-hour care with rotation)nnResponsibilities:nnProvide comprehensive care, including ventilator and tracheostomy management, G-Tube care, and support for daily living activities.nnMonitor the patient’s condition and respond promptly to medical needs.nnCollaborate with the care team to ensure continuity of care and patient safety.nnBenefits:nnStructured Schedule:Rotating 12-hour shifts for comprehensive care coverage.nnOne-on-One Care:Build a strong relationship with the patient and their family.nnWeekly Pay:Competitive wages with weekly direct deposit.nnTraining and Support:Access to resources for ongoing professional development.nnCertifications Required:nnActive LPN license in Tennessee.nnCurrent CPR certification.nnMinimum of two years of ventilator and trach experience required.nnResponsibilitiesnn• Perform selected nursing procedures within the scope of LPN practice• Provide patient and family education according to plan as outlined by the RN• Monitor patient’s vital signs and make specific measurements and observations• Monitor and record intake and output, and report significant changes immediately• Observe, assess and record patient’s physical and mental condition and report any changes• Administer medications as instructed and ordered by the physician and instructed by the RN• Attend and participate in staff meetings and in-service education programs, as assignednnQualificationsnn• Active LPN license in good standing• Previous home health, private duty, skilled nursing or hospital experience preferred• Ability to build rapport with patients and family members• Strong problem solving and critical thinking skills• Reliable transportation or valid driver’s license with current automobile insurancennAbout our Line of BusinessnnAdoration Home Health and Hospice provides quality and compassionate services in the comfort of home, providing support for patients, families and caregivers in their time of need.
