The LPN administers care to patients under the direction of the responsible Registered Nurse or medical provider. To perform this job successfully, an individual must be able to perform each duty satisfactorily.nnEssential Duties and Responsibilities:nnServe as a mentor and resource for nursing standards, competencies, policies and procedures.nnCollaborates with staff and other departments in the development of strategies to continuously improve quality of care and the patient experience.nnUtilize data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.nnProvide direct and indirect care that is safe, ethical, patient and family-centered, and evidence-based.nnProtect patients and employees by adhering to infection control policies and protocols.nnPrepare the patient for encounter by following established protocols.nnDemonstrate competence in clinical skill set.nnDemonstrate proper medication and vaccine management processes.nnReconcile the medication list and prepares prescription renewals.nnDemonstrate complete, accurate, and timely documentation in the electronic medical record.nnRespond to patient phone calls and engage provider or RN appropriately.nnReinforce patient and family education plans that meet the needs of the patient.nnPerform ordered point of care testing (CLIA), including specimen collection, and proper documentation.nnAssist with patient encounter flow and trouble-shooting issues in collaboration with supervisor.nnProvide guidance and mentors new and incumbent staff based on skill set.nnAttend meetings and serves on various organization committees.nnAssist supervisor with compliance and staff development duties.nnAssist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.nnComplete health center daily, weekly or monthly tasks as assigned.nnEnsure operation of medical equipment by following manufacturer’s instructions, troubleshooting malfunctions, and requesting repairs.nnPerform additional duties as assigned.nnQualifications: The requirements listed below are representative of the knowledge, skill and ability to perform the essential functions:nnUnrestricted current LPN license to practice nursing in New York State.nnMust possess a certificate from an accredited school of nursing.nnMinimum of 1-year clinical experience preferred.nnAbility to travel throughout the Network.nnCurrent BLS certification.nnProficiency with Microsoft applications and electronic medical records.nnExcellent verbal and written communication skills.nnSelf-directed, detail-oriented, motivated and ability to work independently as well as in a team atmosphere.nnProficiency with Microsoft applications and electronic medical records.nnExcellent verbal and written communication skills.nnThe pay rate for this position is $28.00/hour.
