We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility team. The LPN will provide high-quality care to residents in a supportive and nurturing environment. The ideal candidate will work collaboratively with other healthcare professionals to ensure optimal patient care, promote resident well-being, and maintain a safe and comfortable living environment for all residents. Key Responsibilities And Qualifications:

n- Resident Care: n
n- Administer medications and treatments as prescribed by physicians. Follow infection control protocols to prevent the spread of illness and ensure a safe environment for residents and staff.
n- Monitor residents' health status, including vital signs, and report any changes to the nursing team.
n- Provide hands-on nursing care, including wound care, catheter care, and assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.

n n- Documentation and Reporting: n
n- Maintain accurate and up-to-date medical records for each resident in compliance with facility policies and regulatory requirements.
n- Assist in the development and revision of individualized care plans for residents.
n
n n- Collaboration and Teamwork: n
n- Work closely with Registered Nurses (RNs), physicians, and other healthcare providers to coordinate patient care and ensure the highest standards of clinical practice.
n- Communicate effectively with residents and their families to provide updates on health status and care plans.
n
n n- Education and Licensure: n
n- Current and valid LPN license in Utah.
n- Completion of an accredited practical nursing program. CPR and First Aid certification required.
n
n n- Experience and Skills:
nn- Strong clinical assessment and critical thinking skills. Proficiency in using electronic health records (EHR) systems.
n- Ability to work independently as well as part of a collaborative healthcare team.
n- Excellent communication and interpersonal skills, with the ability to interact compassionately with residents, families, and colleagues.

n n- Physical Demands: n
n- Ability to lift and transfer residents as needed, with appropriate lifting techniques and assistive devices.
n- Ability to stand, walk, and sit for extended periods of time. Occasional bending, stooping, and reaching may be required.
