JOB REQUIREMENTSnnEducationnnDegree/diploma in nursing and current State of Arkansas Registered Nurse license required. Bachelor's Degree in Nursing preferred.nnExperiencenno A minimum of one year experience working with diverse population and five years case management experience. Recent clinical nursing experience required. Community nursing experience strongly preferred. Sound clinical skills and demonstrated good judgment. Working knowledge of physical, mental and social needs of a diverse population. Ability to problem-solves and plans effectively. Effective oral and written communication skills. Basic computer skills.nnPhysicalnnThis is a safety sensitive position. Please see the St. Bernards Substance Abuse Policy for further information.nno Requires physical strength to perform the essential functions of the job. Must be able to work required schedule. Occasional home visits may be required. Involves being inside and outside. Exposure to temperatures 32 degrees and less and 100 degrees or more, fumes, dust, and noise. Hearing within normal range. Frequent sitting, stooping, standing, walking, bending, pushing/pulling up to 250 pounds, carrying up to 35 pounds, and lifting up to 100 pounds. Must have reliable transportation and current drivers license valid in the state of Arkansas and automobile liability insurance.nnJOB SUMMARYnnCollaborate and communicate with physicians and health care professionals to promote care coordination across the health care continuum for all members of employee group. Serves as clinical advocate to facilitate medical care and navigate through the medical care process. Will work with employee group member, family, and health care team to plan, implement and coordinate services required to meet improved health care outcomes (may include primary prevention, behavior modification programs, and compliance), etc. Member’s plan of care will be coordinated with provider, SHARP and Blue Advantage insurance company. Special emphasis will be placed on high risk members.
