Must have reliable transportation and current drivers license valid in the state of Arkansas and automobile liability insurance.nnJOB SUMMARYnnCollaborate and communicate with physicians and health care professionals to promote care coordination across the health care continuum for all members of employee group. Serves as clinical advocate to facilitate medical care and navigate through the medical care process. Will work with employee group member, family, and health care team to plan, implement and coordinate services required to meet improved health care outcomes (may include primary prevention, behavior modification programs, and compliance), etc. Member’s plan of care will be coordinated with provider, SHARP and Blue Advantage insurance company. Special emphasis will be placed on high risk members.
