Come home to Matrix.nnMatrix Providers is hiring a Licensed Practical Nurse (LPN/LVN) to join our team of talented professionals who provide health care services to our Military Service Members and their families in Jacksonville, FL at Naval Hospital Jacksonville.nnEmployment Status: Full-time positionnnCompensation: This is an hourly position, paid bi-weekly.nnSchedule: Monday- Friday, between the hours of 6:30 am to 6:30 pm. No weekends, no on-call, no holidays.nnBenefits: Competitive financial package with a comprehensive insurance package including health, dental, vision, and life coverage.nnAccrued Paid Time Off (PTO)nnPaid Holidays (Outlined in Handbook)nn401(k) PlannnRequirements: The Licensed Practical Nurse (LPN/LVN) must have and meet the following:nnDegree/Education : Be a graduate from a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program accredited by the Accreditation Commission for Education in Nursing (ACEN).nnCertifications in addition to Basic Life Support : Advance Cardiac LifennExperience: Possess one (1) year of clinical work experience as a full-time LPN/LVNnnwithin the preceding three (3) years.nnLicensure/Registration: Possess and maintain a current unrestricted license tonnpractice as an LPN/LVN in any one (1) of the 50 States, the District of Columbia, thennCommonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.nnJob Summary:nnProvide care coordination using specific criteria and guidelines as mandated by the Department of Defense (DOD), Defense Health Agency (DHA), and TRICARE directives, coordinating with the local Medical Home Port (MHP) clinic, utilization and Referral Management policies, and obtains additional clinical data as required.nnConsult, coordinate, and collaborate with MHP team members, referral management team members, specialty care clinics, outside care professionals, agencies, organizations and other ancillary support systems to assist in the care coordination of MHP patients.nnIdentify patients in need of care coordination within the MHP teams, through screening patients with a pattern of high risk, unstable and resource intensive utilization of Military Treatment Facility (MTF) or network services such as chronic pain patients.nnResponsible for coordination of outpatient care services of patients belonging to the MHP clinic area of responsibility that are discharged from other facilities.nnFollow up with MHP team members regarding review of patient charts, medical records, and other documents utilized in the treatment and care of MHP patients in need of care coordination.nnParticipate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program.nnParticipate in tele-mentoring, tele-medicine, and tele-consults.nnMatrix Providers is an equal opportunity employer.
