Job Description Summary Exciting and challenging part-time opportunity as an integral part of the multidisciplinary Inpatient Palliative Care Team providing clinical social work services to patients and their families at South Shore Hospital/South Shore Health, in South Weymouth, MA. This social worker position will provide support for patients and families who are coping with serious illnesses as well as the accompanying life changes as they anticipate potential worsening clinical conditions, death and bereavement. The role on the Palliative Care Team calls for psychosocial assessment and intervention to address patients/family's needs and concerns, crisis intervention, consultation, and the ability to work collaboratively within a multi-disciplinary team clinical framework. LICSW Required Job Description Job Responsibilities: Complete and document the psychosocial portion of the multidisciplinary assessment of patients in accordance with established standards of practice. Assessment will include comprehensive data on the person with the current medical issues and community related factors. Develops and implements treatment plans for appropriate patients in accordance with established standards of care and practice. Treatment plan will be developed which reflects an integration of clinical social work practice. Treatment plan will be presented and discussed with patient as appropriate. Completes all documentation of services according to established standards. Documents patient status as observed during clinical session. All documentation will occur at time of the patient interaction. Provides psychosocial and emotional support to patients and families. Facilitates communication between family members to achieve a level of emotional support as needed for patient's medical progress. Provide end-of-life support and education to patients and families coping with death and dying. Documents counseling in patient chart at time of interaction. Identifies psychosocial issues for team members and aids in the understanding of how these issues affect patient/family and treatment planning. Effectively communicates with team members regarding patient status to facilitate interdisciplinary assessment and treatment. Attends and participates in at least 80% of regularly scheduled Team meetings. Demonstrates a thorough knowledge of community resources and functions as a patient advocate. Establishes and maintains a comprehensive resource file. Empowers and effectively represents the patient, as needed, in order to secure available services. Actively functions as a member of the patient care team to collaborate in the formulation and implementation of plan of care. Communicates with nursing, case management and Providers, as needed, regarding mental health issues and social service needs. Facilitates integration of social service goals into a cohesive team treatment plan. Identifies patients in need of crisis intervention and management. As a Mandated Reporter, demonstrates knowledge of family violence, child/elder/disabled neglect and abuse so as to identify those at risk and refer them to appropriate agencies per Hospital guidelines, as evidenced by documentation in the clinical record. Collaborates and facilitates case conferences with appropriate members of the interdisciplinary team to assess need for reporting to appropriate agency, implements changes in plan of care in a timely fashion and completes all required paperwork with date, signature and title. Education: Master's Degree from a CSW accredited school of social work preferred. Work Experience: Two (2) years’ experience in an acute hospital setting preferred.
