Requirements:
- Board Certified - REQUIRED if Board Eligible (ABIM ONLY)
- Active WA license or IMLC (LOQ in hand/must upload during name clears) - REQUIRED
- Completed Endocrinology Fellowship - REQUIRED
- Malpractice - Clean Preferred
- Availability at time of name clear - REQUIRED
- ACLS/BLS/DEA - REQUIRED
