• Board Certified
• State License - ND License is preferred - IMLC is okay
• Residency Requirements/Fellowship - REQUIRED
• ACLS - REQUIRED
• Malpractice - Clean Preferred
• Availability at time of name clear - REQUIRED
• Must have experience with all required procedures
