Experience:
- 2 years of med/surg experience
- Wound and colostomy experience required
License:
- Valid New York State RN License
- Primary Source Verification
- AHA BLS certification
Certifications:
- Certified Wound Ostomy Continence Nurse (CWOCN) issued by the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB)
- Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB
- Certified Wound Care Nurse (CWCN) issued by the WOCNCB
- Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM)
- Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO)
Must-Have:
- Wound Care Certification

Job Description:
- Conduct health assessment interviews focused on wound, ostomy, and continence care.
- Perform focused physical assessments and develop care plans based on diagnosis and client needs.
- Recommend medications and treatments as appropriate.
- Develop, revise, and evaluate standards of care, assisting in selecting supplies, technology, and equipment.
- Assess and document patient response to wound care, ostomy, continence, fistula, and complex wounds treatment.
- Evaluate the quality and delivery of care provided by clinical staff.
- Collaborate with the healthcare team and make referrals to other disciplines as needed.
- Teach, counsel, and assist individuals and families in preventing complications related to ostomies, wounds, or continence problems.
- Develop, provide, and evaluate staff education programs, such as new employee orientation, in-services, and continuing education programs.
- Conduct comprehensive assessments of patients at risk for developing pressure injuries, and initiate preventative measures.
- Communicate and escalate challenges, including ethical issues and conflict resolution.
- Collaborate with the Assistant Director of Nursing for Wound Care to recommend best practices.
- Participate in Quality Improvement activities, including incidence and prevalence studies, and nursing quality initiatives.
- Provide culturally competent patient, family, and staff education consistent with belief systems.
- Maintain the security and confidentiality of data and information in accordance with patient rights policies.
- Achieve and maintain a level of professional competency through engaging in activities consistent with clinical practice.
- Identify community resources helpful to clients and assist them in utilizing such resources.
- Manage assets related to rented equipment, such as specialty beds and negative pressure devices.
- Travel to patients' homes to complete wound care consults when needed, handling an average of 15 to 20 consults a day.
