Title: Care Manager

Remote

3 Month (C2H)

Monday–Friday, 8 AM–5 PM.

• This role is 80–90% remote, with occasional in-person meetings to connect with members and their families in Osceola, Orange, Or Seminole counties .
• Quarterly meetings will be held at our office in Orlando, FL (6801 W Colonial Drive, Suite E) .

Key Responsibilities

• Conduct daily outreach calls (5–20 per day) based on member-specific tiers to address needs and remove barriers to care.
• Assess, plan, and coordinate medical and support services to promote cost-effective, quality care.
• Act as a supportive liaison for families, focusing on connecting members to appropriate resources.

Preferred Background

• 2+ years of experience in social work, behavioral health, or community care.
• Familiarity with government-sponsored managed care programs.
• Experience connecting families to resources for housing, employment, foster care, or justice services.

Qualifications

• Education : Bachelor’s degree required.
• Licensure : Social Work Licensure in Florida or Compact Licensure is required. OR Registered Nurse (LCSW/LMSW/RN)
• Preferred : Pediatric case management experience, health plan experience, foster care/adoption background, bilingual proficiency.
