Sentara Medical Group has a full-time opening for an experienced APP (NP or PA) to join our Ambulatory Remote Patient Monitoring team in providing primary care.
This is a new program in partnership with HealthSnap that is focused on preventive measures to help patients manage chronic conditions such as high blood pressure, diabetes, etc. with a goal of preventing admissions and readmissions to the emergency department.
Position Highlights:
- 100% remote
- 8am-5pm, no nights or weekends
- Provides collaboration in a team-based environment with other primary care providers and HealthSnap employees

Minimum Requirements:
- Active NP or PA
- Must have experience practicing as an APP (3 years preferred) managing own patient panel and the ability to work independently
- Telehealth experience is a plus
- Applicants may live in the following states: Indiana, Nevada, South Carolina, South Dakota, Tennessee, Texas, Virginia, West Virginia, or Wisconsin
