Job Description

The primary purpose of this position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s). Performs assessments in collaboration with the physician and as needed. Develops and implements appropriate plans of care to PO participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participants caregivers, as well as documenting changes in a participants medical record consistent with documentation policies.

Nurse Practitioner 

Status: Full time 

Shift:  Mon to Fri, 8 am to 430 pm 

On Call: Required for after hours and weekend coverage on a rotating basis

What you will do: 

  • Provide primary medical coverage in collaboration with PO physicians as needed. Performs initial, semi-annual and annual assessments, performs additional assessments of participants as needed and implements appropriate plans of care to PO participants. 
  • Participates in discharge planning for acute and long-term placement. 
  • Evaluate and treat participants during acute illness. Prudently prescribes medications, therapies and other treatments for participants. Following consultation with the Primary Care Physician refers participants to medical specialist as indicated.  
  • Make nursing home/or home visits when needed. 
  • Work with Medical Director and Clinic Supervisor to formulate clinical policies, procedures and standards of care.  Assists with the development of standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues. 
  • Advise the Medical Director and Primary Care Provider in ways and means to establish better accountability of the PO services to participants and referral sources. Acts as resource during intake of new participants and in the day-to-day operation of the center programs. 

Minimum Qualifications: 

  • Master's degree in Nursing 
  • Five (5) to seven (7) years of experience required preferably in long-term care.  
  • Must possess a current, unencumbered, active license to practice in the state of PA. 
  • CDS Certification and DEA Registration required.  
  • Board Certification by ANCC or AANP.  
  • Certification as a Nurse Practitioner with prescriptive authority required.  
  • Current BLS required.  
  • A minimum of one (1) year working with the frail or elderly population strongly preferred 
  • Must be action-oriented, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility. 
  • Must have a valid drivers license, proof of insurance and have means of transportation 
  • Proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system 
  • Position requires periodic travel within the Centers geographic region. 
  • Position requires frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
