FOR MORE INFO CLICK HERE: National Health Service CorpsRole:The Complex Care Nurse (CCRN) for Substance Use Disorder (SUD) Care collaborates with a multidisciplinary team of medical providers, primary care nurses, clinical pharmacists, behavioral health professionals, and other community health sites to care for patients with substance use.Utilizing a harm reduction approach, the CCRN for SUD Care provides clinical leadership and skilled nursing services, including triage, assessment, case management, education, and outreach. The role involves ongoing assessment, intakes, evaluations, teaching, initiations, stabilizations, and monitoring of patients. The CCRN for SUD Care is also responsible for managing patient care plans, performing crisis intervention and referral management.The position may require intermittent and timely travel between multiple sites and flexible hours based on patient needs. Compliance with Massachusetts State Laws and regulations set forth by the Department of Public Health and Harbor Health Services, Inc. is essential.Responsibilities: Conducts initial assessments and intakes, gathering histories including social, medical, mental health, substance use, and medication history.Triages and provision of low-barrier access for all phone and walk in inquires for treatment.Reviews assessments with the SUD clinical team to determine appropriate care plans.Manages patient care plans, performs crisis intervention and management of referrals.Collaborates and communicates with the SUD team, attending meetings as required.Provides ongoing education, support, and service coordination for patients.Serves as liaison and provides education and support for primary care teams and community partners such as hospitals, VNA, lab, or other medical offices, outreach and teamsAssists in creating clinical forms and ensures meticulous documentation.Collects pertinent data and specimens, reviews commonly found results with patients, taking clinical actions per protocol.Administers Immunizations as requested by providers or per protocol and accurately documents in the EMR. Coordinates with the pharmacy and SUD Care team on medication orders, medication administration, prescriptions, and refills.Counsels patients on relapse prevention and pharmacotherapy and provides referrals and resources for non-pharmacotherapy as indicated. Provides follow-up care, scheduling clinical visits and phone support.Engages in primary care and addiction-related projects.Collects and submits required data for the Bureau of Substance Abuse Services.Assists with phone triage, prescription refills, follow-up test results, and managing medical issues.Assists providers in the provision of care: procedures, injections, EKG’s, referrals, prior authorization, specialty pharmacy, security and documentation of controlled substances.Performs all nursing duties in accordance with Massachusetts Nursing Practice Act, while adhering to Harbor Health policies and procedures.Follows HHSI policy when disposing of biohazardous materials, labeling and handling all materials.Requirements: Successful completion of a nursing program (RN), BSN preferred Current Massachusetts Licensure as Registered Nurse (RN) and current BLS (Basic Life Support for Healthcare Providers) Certification requiredCertified Addictions RN (CARN) required or obtained within 1 year of employment2 years of experience in a substance use disorder environment, working with a substance abuse population, or in an outpatient setting with a focus on addictions and/or behavioral health counselingExperience or training in de-escalation preferred2-4 years of nursing & primary care experience preferredExperience in community health with a focus on cultural sensitivity and the ability to work comfortably with diverse economical, racial, multi-linguistic and ethnic populationsBasic to Intermediate knowledge of Microsoft Office 365 including Word, Excel, Outlook and Teams, OCHIN/EPIC Electronic Medical Record (EMR) experience preferredBilingual English and Vietnamese, Brazilian Portuguese, or Spanish preferred but not requiredAdvanced interpersonal skills (communication and education) necessary to interact effectively with all levels – patients and their families, other healthcare teams (external and internal)Requires knowledge of processes and community resources and agencies relative to health, protective services, crisis intervention, disability, etc. to aid in adjusting or tapering controlled substancesWillingness to train and orient new personnelDemonstrates flexibility in meeting patient needs and accommodating changes in clinical schedulesStrong patient advocate and an anti-stigma change agentAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or veteran status Sign on bonus offered , one half on hire and remainder after 90 days in good standing, former employees of Harbor Health not eligible.
