Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.nnResponsibilities:nnResponsible for nursing care under the supervision of physicians and/or Registered Nurse (RN) and within the appropriate scope of practice. Performs the functions associated with the clinical aspect of a patient visit in Primary Care. Maintains Responsibility for direct patient care, documentation, administration of medications and injections, minor laboratory testing, and communication with patients, health care providers, insurers, pharmacies and fellow staff. Occasional evening and weekend work is possible.Responsibilities This position will require travel to new builds and acquisitions GENERAL PURPOSE: Responsible for nursing care under the supervision of physicians and/or Registered Nurse (RN) and within the appropriate scope of practice. Performs the functions associated with the clinical aspect of a patient visit in Primary Care. Maintains Responsibility for direct patient care, documentation, administration of medications and injections, minor laboratory testing, and communication with patients, health care providers, insurers, pharmacies and fellow staff. Occasional evening and weekend work is possible. JOB DUTIES AND RESPONSIBILITIES: - Direct Patient Care: Escorts patients to exam rooms. Reviews patient charts to anticipate health needs and organize flow. Obtains and records patient vital signs. Provides direct patient care services per scope of primary care practice. Performs required tests at the direction of the provider. Assists providers in performing exams, diagnostic procedures and treatments. Gathers and relays instructions and information between providers, patients and families. - Telephone Management: Returns patient calls per direction of provider. Collects patient information and communicate with providers for direction. Works in tandem with secretaries to schedule office and referral appointments. Handles prior authorization/referral information to insurers. - Administration of Medications and Injections: Provides basic care as ordered by providers. Administers medications and treatments as directed and per scope of practice guidelines including injections (allergy, immunizations, and therapeutic). Reviews medications at time of visit as directed by provider. Maintains inventory of and order medications and medical supplies with attention to expiration dates and infection prevention guidelines. - Electronic Recordkeeping: Documents interventions, patient/family responses, medications dispensed/prescribed, and testing results in electronic medical records (EMR). Monitors and follows up on electronic messaging promptly and accurately with appropriate documentation. - Compliance: Performs quality assurance and CLIA waive testing as directed. Comply with all PCA policies and procedures. Complies with standard HIPAA, OSHA and JCAHO regulations. - Patient Education: Promotes wellness by providing materials and communicating physician advice/instructions. Provides patient education at the direction of the provider and/or RN. - Laboratory Testing: Collects specimens for laboratory analysis and accurate recording of lab requisitions. Records and maintains appropriate laboratory logs. - Other duties as assigned. QUALIFICATIONS: Graduation from nursing education program acceptable to NYSED required. Six months of experience and outpatient primary care and/or pediatric (if applicable) experience preferred or equivalent combination of education and experience. Direct patient care experience, medication and vaccine administration experience, electronic medical records knowledge preferred. Certification as a Licensed Practical Nurse in New York State, CPR (BLS for Healthcare Provider) and AED certification (AHA) required.
