Experience: 1 year of experience in an LPN capacity at an LTC facility
License: NY LPN Licensure
Certifications: BLS (certified through AHA or RQI specifically)
Must-Have: Experience and willingness to work every other weekend (EOW) and on holidays
Job Description:
- Demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit.
- Understand principles of growth and development across the lifespan and possess the ability to assess data reflective of the patient's status.
- Provide care to adult and geriatric patient groups, tailored to their age-specific needs.
- Deliver direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with nursing staff, physicians, and ancillary staff.
- Pursue continuing education and participate in in-service programs to maintain clinical expertise and knowledge.
- Collect and communicate data relevant to a resident's physical and psychosocial condition to the RN and Interdisciplinary Team.
- Assist in creating and revising developmentally and culturally appropriate care plans to achieve desired resident outcomes.
- Engage resident/family and healthcare team in the care process while promoting a safe environment.
- Provide leadership in the professional practice setting under the direction of an RN.
- Consider safety, cost, and impact in planning and delivering care.
- Participate in mentoring and orientation of staff.
- Anticipate occupational exposure to bloodborne pathogens, with tasks involving potential contact with blood, body fluids, or tissues.
