Your Specific Responsibilities:

n

This employee Is an experienced Registered Nurse Specialist, who applies nursing principles and techniques in the care and treatment of clients in public health settings and on home visits if applicable. Conducts specialty clinics as indicated. Works under the direction of the Assistant Community Health Nursing Director. The incumbent will rotate to other clinical sites and programs within DOH-Lake as the primary registered nurse for the HIV/AIDS program. This employee will interview and counsel clients, obtains pertinent medical history, assesses client's health status, immunization status, evaluate needs and performs necessary clinical testing. Educates clients in specific health needs, medications, and vaccines. Assist the provider as needed and conduct exit interviews to assure continuity of care and client understanding of education and counseling providing during the clinical visit.

Performs all necessary nursing functions in support of the HIV/AIDS clinical program at the Florida Department of Health in Lake County including coordination of medical and preventative services for clients based on need and evidence-based guidelines. Monitor and follow up of benchmarks for client's on-going medical and treatment plan such as compliance with labs, medication, and primary and specialty care referral appointments. Provides education and counseling of health principals and risk behaviors, adherence, nutrition, and prevention. Enters all client information into clinical databases including referrals, follow-up, and clinical visit documentation to ensure continuity of care.

Maintains proper and necessary records of each client in all electronic health record databases including monitoring and follow up of abnormal test results facilitating referrals to other health care and/or social agencies to assist with services and treatment various medical issues. Ensures completion of all clinic visits components in all Electronic Health Record Systems to include but not limited to medications, vaccines, education components, and referral documentation. Reviews and logs medical records from outside providers, creates progress notes to document telephone calls for follow-up on client activities. Demonstrates skill in use of medical equipment, knowledge of infectious disease specific laboratory tests and the administration of prescribed medications specific to STD/HIV treatments as ordered.

Conducts nurse clinic under the direction of the clinical provider. Administers immunizations, medications, and treatments within the established CHD protocols. Inputs in Fl Shots as vaccines are given and ensures input into EMS. Responsible every day to examine and assure compliance with HMS and Florida Shots and the inventory corresponds at 100%. Maintains knowledge of immunizations for adults and children. Ensures that all medications and inventory logs are at 100%, daily. Maintain an accurate medical supply inventory. Responsible daily, to enter into medical inventory book, all medications and vaccines issued, scan medications out as appropriate and to be at 100% compliance daily.

Participates in Clinical Quality Management processes related to STD/HIV related issues for residents within DOH-Lake. Participates in approve program specific STD/HIV meetings and conferences.
