Compensation:
Salary Range - $79,040.00 - $93,600.00. Compensation may vary within the salary range based on factors such as experience, education, skills, and licensure.

As a Hospice RN / RN Case Manager, you will:
- Report directly to the Administrator or Patient Care Manager and manage the professional care of each patient.
- Act as a primary liaison between the physician, patient/family, and hospice team.
- Deliver direct patient care services and provide supportive care to families, maintaining a reasonable caseload and a compassionate relationship.
- Empower caregivers through education on how to care for their loved ones.
- Collaborate with an interdisciplinary team to ensure comfort, peace, and dignity for patients.
- Assess patient and family needs and help prepare and implement the care plan.
- Maintain all clinical records for assigned patients.
- Engage in quality assessment and performance improvement initiatives.
About You:
- Compassionate, kind, and empathetic approach towards patients and families.
- Willingness to provide care in various home settings.
- Ability to stay flexible and composed during crises.
- Active RN license in the state of employment (or eligibility to obtain one).
- 1+ years of nursing experience in hospice, home health, ICU, geriatrics, or oncology preferred.
- Valid driver's license and automobile insurance required.
- Current CPR certification is necessary.
