As a MDS Coordinator, you will maintain the standards of nursing care and implement policies and procedures of the facility and the nursing department. Responsible for managing the interdisciplinary team process including the development of the care plan based on the completed Minimum Data Set (MDS) to assure regulatory compliance and address special needs of each resident.nnMDS Coordinator Responsibilities:nnCompletes assessments, Minimum Data Set (MDS) and care plans for all residents assigned.nnMonitors completion of MDSs by other disciplines within time frames prescribed by regulatory guidelines.nnAdvises supervisor of incomplete and/or untimely assessments by disciplines other than nursing.nnInitiates care plans and supporting activities that will result in best possible outcome for assigned residents.nnGenerates and distributes monthly care plan calendar for the following month.nnConducts care plan conferences for assigned residents.nnMaintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues as necessary.nnReviews all resident incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs.nnAt patient care conferences, facilitates and coordinates the activities of the disciplines of nursing, rehabilitation, dietary, activities, social services and restorative nursing.nnMonitors and ensures completion of 7-day look-back documentation by nursing staff within prescribed MDS time frame.nnParticipates in facility management meetings as requested.nnMonitors and audits clinical record documentation on assigned residents to ensure accuracy and timeliness.nnPromotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current / new regulations.nnMaintains current knowledge MDS assessment process.nnEXPERIENCE WITH PPS/CASE MIX PREFERRED.nnEXPERIENCE WITH MDS 3.0 PREFERRED.nnWe look forward to hearing from you!!nnIND123
