Job Description: UPMC Hamot in Erie, PA welcomes you to join us as a full-time Pharmacist Our role will serve variable days and evenings, 7:00a.m. - 3:30p.m. and 12:30p.m. - 9:00p.m.; our future teammate will work every third weekend and 2-3 holidays each year. Along with an exciting role, we are happy to offer professional growth opportunities as well as a comprehensive Total Rewards benefits package to help you achieve your goals because we believe you’re at your best when receiving the support you need in all areas of life: professional, personal, financial, and more.Responsibilities:\n\n\n Interpret physician orders and prescriptions to accurately prepare, calculate, and dispense medication dosages for both inpatient and outpatient settings, adhering to the Pennsylvania Pharmacy Practice Act and federal regulations.\n\n\n Identify and promptly report suspected adverse drug reactions to ensure patient safety and improve therapeutic outcomes.\n\n\n Sustain the medication formulary by minimizing non-formulary procurements, implementing therapeutic interchange protocols, and promoting rational drug therapy to optimize patient care.\n\n\n Screen and maintain patient medication records for allergies, drug interactions, and therapeutic duplications, ensuring the accuracy of orders during verification processes.\n\n\n Organize daily pharmacy operations and assist in supervising pharmacy technicians, aides, interns, and other personnel to enhance pharmaceutical care delivery.\n\n\n Adhere to all regulatory requirements, professional standards, and hospital policies to ensure high-quality pharmaceutical care.\n\n\n Communicate effectively with physicians and nurses to clarify unclear, illegible, or incomplete orders, fostering collaborative healthcare.\n\n\n Provide drug information and educational programs to healthcare professionals and pharmacy students while leveraging advanced computer systems and technologies in medication management.\n\n Required:\n\nGraduate of an ACPE accredited College of Pharmacy\nB.S in Pharmacy or Doctor of Pharmacy (PharmD) degree\nCurrent licensure as a Pharmacist in practicing state\n\n Preferred:\n\nPharmacist experience\nStrong written and verbal communication skills\nExcellent communication skills\n\n Licensure, Certifications, and Clearances:\n\nPharmacist\nAct 34\n\n UPMC is an Equal Opportunity Employer/Disability/Veteran
