Job Description: RN Unit MANAGER - NEW YORK - BROOKLYNThe RN Unit Manager (Registered Nurse) coordinates medically necessary services to ensure effective organization of long-term care and health-related care across various community, home, and facility-based settings.Performance Responsibilities and StandardsProvide care management to a group of members through collaborative assessment, planning, facilitation, and advocacy for options and services, utilizing available resources.Implement quality and cost-effective care plans in collaboration with members, families, healthcare teams, and community programs.Revise care plans based on changes in members' physical, mental, or social well-being, developing comprehensive individualized care plans.Utilize evidence-based strategies to prevent hospitalization and inappropriate ER utilization.Engage members and families to optimize wellness, self-care, and functional capabilities.Oversee managed care caseload, evaluating assessment data and clinical notes.Complete monthly member phone contacts following agency standards.Facilitate timely referrals to health and community-based resources.Document all member activity accurately in Medical records.Review and authorize service requests according to workflows and timelines.Collaborate with members, Primary Care Providers, interdisciplinary teams, caregivers, and families.Attend care conferences with the Supervisor.Promote quality cost-effective outcomes.Maintain members in independent living situations.Ensure consistent care along the healthcare continuum through continuous assessment.Act as a liaison and advocate for members.Demonstrate cultural competence respecting cultural health beliefs and language needs.Provide services and coordinate within a managed care system.Thoroughly document communications with providers, members, families, and network providers.Ensure compliance with State and Federal regulations.Participate in Quality Assurance and Improvement initiatives.Perform other assigned duties.RequirementsRegistered Professional Nurse license and registration in New York State.Education: BSN preferred; Associates Degree required.One year of Nursing Experience preferred.Experience in long-term care and/or managed care required.Familiarity with Home Healthcare, Medicare, and Medicaid preferred.Preferred Case Management Certification.Bilingual skills are desirable.Ability to work independently, handle multiple assignments, and prioritize.Proficient in basic computer skills, Microsoft Office programs.Strong in-person and phone communication skills.Willingness to learn an organization's software system.Physical RequirementsDesk work comprises 90% of the day.Adequate visual and hearing abilities for communication.Proficiency in verbal and written expression.Comfortable using the telephone.Frequent laptop usage for data entry and information retrieval.Bloodborne Pathogens Exposure Category III - Minimal contact with fluids.Compensation & BenefitsCompetitive Salary With401k, generous Employer matchMedical, Dental, Vision plansBSN Tuition Reimbursement after 6 monthsWork/life balance Employee Recognition PlatformPaid Time Off, Holidays, Extended Sick LeaveShort/Long term DisabilityEmployee Assistance Program (EAP)And MoreIf you are dedicated to patient care, team developmentseek work-life balance, and attractive benefits, email your resume to Christian Hanson at christian.hansongogpac.com.
