Job Description: One of our Clients, is looking for a Nurse Practitioner.Location: New York/New JerseyPosition Type: Full-Time/PermSchedule: 8 Hour Shifts On-siteResponsibilitiesPatient Care Management: Conduct comprehensive assessments, including medical history reviews and physical examinations for patients of all ages. Diagnose and manage acute and chronic conditions, including ordering and interpreting diagnostic tests. Develop and implement individualized treatment plans in collaboration with patients and families. Health Promotion And Disease Prevention Educate and counsel patients on health maintenance, disease prevention, and lifestyle modifications. Administer vaccinations and perform routine screenings as part of preventive care. Address and manage risk factors such as obesity, smoking, and high blood pressure. Clinical Procedures Perform minor procedures such as suturing, wound care, and joint injections. Prescribe and manage medications while ensuring adherence. Coordinate referrals to specialists and other healthcare providers as needed. Collaboration And Team Leadership Work closely with physicians, nurses, and administrative staff to ensure coordinated and seamless patient care. Participate in team meetings and contribute to clinical decision-making processes. Mentor and provide education to nursing staff or students as applicable. Patient-Centered Approach Build trusting relationships with patients to encourage open communication. Address patients' cultural, social, and emotional needs in care plans. Maintain patient confidentiality and adhere to HIPAA regulations. Qualifications Master's or Doctorate in Nursing (MSN/DNP) with specialization in Family, Adult-Gerontology, or Primary Care. Active Nurse Practitioner license and certification (e.g., FNP, ANP, or AGNP) in NY and NJ. Proven experience in a primary care or family medicine setting is preferred. Proficiency in conducting physical examinations, diagnosing common illnesses, and managing chronic conditions. Familiarity with electronic medical records (EMR) systems and basic computer applications. Excellent diagnostic, clinical, and decision-making skills.
