Community Options, Inc.  is a national non-profit agency providing services to individuals with disabilities in 12 states.   We are now hiring a Full-Time Physical Therapist in Hillsborough, NJ. The Physical Therapist will provide medical, therapeutic and recreational support to individuals with developmental disabilities.   Responsibilities

• Responsible for providing rehabilitating services to our individuals
• Evaluate consumers, create physical treatment plans, and ensure the implementation of the plan
• Responsible for training staff on all plans
• Ensure that all consumers' adaptive equipment are operable
• Serve as an active member of each person’s Inter-Disciplinary Team (IDT), which includes involvement in Individual Habilitation Plan development
• Prepare progress reports for consumer's immediate support group
• Ensure that staff are documenting accurately and current physical therapy plans are being implemented, to ensure our consumer’s overall needs are being met
• Comply with all internal and state regulations and participate in all inspections and investigations
• Additional tasks and responsibilities may be assigned
• Employee must cooperate with the licensee and department staff in any inspection, inquiry, or investigation
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing

Minimum Requirements

• Bachelor's degree in related field
• Minimum 2 years' experience in-home community-based service or related field
• Knowledge and understanding of local regulatory agency operations
• Understanding and commitment to community-based support for persons with disabilities
• Valid?driver's license
