  • JOB SUMMARY
    • Management of patient care across the continuum to insure excellent patient outcomes and appropriate utilization of resources.
  • JOB SPECIFICATIONS
    • Education and/or experience: Licensed Registered Nurse in the State Of Tennessee; 2 years Acutecare Setting, Certification In Case Management preferred but not required.
    • Knowledge: Case Management standards; utilization management criteria
    • Skills: Good organization and communication skills; computer skills
    • Abilities: Ability to interact with patients/families and other healthcare disciplines to coordinate seamless continuity of patient care
