Job Summary: The FRPS Mid-Level provider (APRN) will be responsible for the comprehensive clinical evaluation of a wide variety of medical patients while under the supervisory capacity of the attending Physician. The clinical evaluation will take place in the context during daily patient rounds for those patients previously admitted for care.

Essential Job Duties and Responsibilities:
1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served for the following age groups:

2. Performs all responsibilities/duties required by Faith Regional Physician Services as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes but is not limited to the age of the patient served.
• Demonstrates accuracy, competency and thoroughness in history taking, assessment and evaluation of the patient's age specific needs (with appropriate differential diagnosis).
• Appropriately initiates medication, lab and x-ray orders.
• Appropriately interpret lab and x-ray results.
• Collaborates with ER physician on evaluation, assessment and treatment plan for all patients, both Select care and ER level.
• Demonstrates appropriate technical aspects of patient care and expertise in technical (such as suturing, splinting.)
• Initiate the discharge plan as appropriate and provide continuity of care to meet the patient's changing needs and achieve the outcomes.
• Includes and communicates the plan of care to patient, family, significant others and relays pertinent information thorough appropriate documentation and referral to other health care team members.
• Records information on the patient record in accordance with charting and regulatory guidelines.
• Recognizes changes in patient status based on interpretation of assessments and carries our physician's orders.
• Performs technical aspects of patient care according to established policies and procedures.
• Delegates and assigns appropriate patient care.
• Abuse Protection: Identifies high risk, dependent relationship and actions to prevent further infliction of physical or emotional harm
• Provides lifesaving measures in life threatening situations.
• Establishes priorities of patient care for urgent treatment
• Comprehensive assessment of the patient's clinical condition with attention focused on the reasons for presentation.
• Diagnostic determinations (Laboratory, Radiological, and other tests) to define the optimal treatment plan.
• On-going treatment and clinical management.
• Referral for appropriate sub-specialty consultations.
• Prescribing medications and other therapeutic interventions.
• Patient and family education and support (including family meetings as required.)
• Determination for readiness and subsequent planning for discharge from the hospital (working closely with case management and utilization review.)
• Update the patient "hand-off" form (for those patients managed by you on a given day) in order to clearly communicate the patient's current clinical status, plans for care and items to be addressed by the next hospitalist on duty.
• Attend monthly hospital medicine staff meetings and participate in specified hospital committee meetings.
• Participate in departmental quality improvement projects and other similar initiatives.
• Document (dictation) all patient encounters in a clear and well-organized fashion, including admission histories, daily progress notes, consultations, procedural notes, code blue/RRT responses and discharge summaries.
• Maintain all health information charts without deficiencies.
• Provide appropriate coding for each patient encounter.
• Demonstrates leadership qualities (characteristics include a strong self-image, vision of the future, a firm belief in the organization, ability to influence others positively).
• Works to maintain positive team relationships with co-workers, physicians, and other members of the healthcare team.
• Demonstrates the ability to problem solve and offer solutions.

• Attends continuing education programs in order to maintain licensure. Attends organization sponsored training programs as required.
• Maintains certifications as determined by department/clinic policy.

6. Completes all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job-related hazards.
7. Understands and adheres to Faith Regional Health Services Corporate Compliance Plan, Personal Conduct Policy, Safe Environment policy, Grievance Process, Confidentiality,
Safety & Security, Infection Control, HIPAA and other departmental and organizational policies, procedures and guidelines.
8. Must be able to remain awake and alert during working hours. Physical presence at facility is required during scheduled working hours to perform job functions.

Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

Current, valid Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license ?
Current Registered Nurse (RN) license or temporary permit from the State of Nebraska
Current Licensed Practical Nurse (LPN) license or temporary from the State of Nebraska
Current Certified Nurse Aide (CNA) license from the State of Nebraska
Basic Life Support (BLS) obtained within one full month from date of hire ?
Advanced Cardiac Life Support (ACLS) obtained within one year from date of hire ?
