One of our top clients is seeking a dedicated and patient-centered RN Case Manager to join our team. In this role, you will play a vital part in ensuring our residents receive the highest quality of care and experience a fulfilling lifestyle.
About You:
  • A registered nurse (RN) with a current Oregon license.
  • Minimum 1 year of experience in long-term care or a related field (preferred).
  • Strong assessment and care coordination skills.
  • Excellent communication and collaboration skills with residents, families, and healthcare professionals.
  • Compassionate and patient-centered approach to care.
  • Proficient in documentation and medical record keeping.
  • Ability to work independently and as part of a team.
What You Will Do:
  • Conduct comprehensive resident assessments to develop individualized care plans.
  • Coordinate care with physicians, therapists, social workers, and other healthcare providers.
  • Educate and empower residents and families regarding their care options.
  • Monitor resident health and well-being, reporting any changes to the healthcare team.
  • Advocate for residents' rights and ensure their quality of life.
  • Maintain accurate and up-to-date resident records.
  • Participate in interdisciplinary team meetings and collaborate to achieve resident goals.
