Job Description

Brightstar Care of St. Paul is growing again! We are currently looking for a compassionate, caring and self-starter Occupational Therapist to provide quality care forpatients in our community. We currently have an opening for a per diem Home Care Occupational Therapist to build the foundation of our newly offered service in Ramsey, Hennepin, and Dakota Counties. Come grow with us! Qualifications: • A graduate of an OT Program recognized by the Accreditation Council for Occupational Therapy Education (ACOTE) and has successfully completed the entry-level certification examination for occupational therapists created and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT). • An active license to practice Occupational Therapyin MN • A minimum of three(3) years’ experience as an Occupational Therapist, home care experience a plus • Must have criminal background check • Must have current cardiopulmonary resuscitation (CPR) certification and/or a Basic Cardiac Life Support (BCLS). • Must have active driver's license and clear driving record • Computer skills. • Ability to exercise initiative and independent judgment Responsibilities: • Assessment, planning, implementation, and evaluation of all occupationaltherapy modalities to patients. • All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization. • Completes all patient evaluations and develops the OT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to organizationwithin 24 hours of the evaluation. • Follows the plan of care by leading the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety. • Returns the individual to optimum and productive level within patient’s capabilities. • Prepares a written discharge summary and/or a written summary report on the patient's condition • Consults with family and agency personnel. • Appropriately utilizes ICD-10 codes when needed. (We are a non-medicare certified agency) Working Environment: Works indoors in Agency office, patient homes and travels to/from patient homes.
