Position Summary:

The primary purpose of the RN Case Manager position is to provide ongoing support and expertise through
comprehensive assessment, planning, implementation, coordination and overall evaluation/monitoring of
individual patient needs. The overall goal of the position is to enhance the quality of patient management and
satisfaction, to promote continuity of care and cost effectiveness through the functions and integration of case
management, utilization review and management and discharge planning.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Previous hospital nursing and/or Case Management experience preferred.

Minimum Qualifications:

•  Associate’s degree in nursing Required 
• Bachelor’s degree Preferred
• (Utah, United States) Registered Nurse
• Must have valid Utah nursing license or compact state license.
• Basic Life Support (BLS) within 60 days. If past due, employee will have 60 days to bring to date.
