General Summary of Duties: We are looking for a dedicated Registered Nurse Case Manager to provide intermittent nursing services to patients in their homes, collaborating closely with physicians to ensure high-quality care. This role supports patients' health and well-being through comprehensive assessments and tailored care plans.
Supervision Received: Reports directly to a Clinical Manager.
Supervision Exercised: Home Care Aides.
Education: Graduate of an NLN-approved school of nursing, with an associate or baccalaureate degree preferred.
Experience: Minimum of one (1) year recent medical/surgical nursing experience required; community health nursing experience is a plus.
Certificate/License: Valid Registered Nurse license in the State of Connecticut.
Typical Physical Demands: This position involves full body motion including lifting, manual dexterity, and the ability to stand, walk, and navigate stairs. A motor vehicle is needed for travel, with occasional lifting of up to 30 pounds required.
Work Conditions: The role is primarily performed in patients' homes and an office environment, with some exposure to communicable diseases and other health-related hazards.
Knowledge, Skills, and Requirements:
- In-depth knowledge of nursing theory and practices.
- Ability to evaluate patient care and implement agency policies.
- Familiarity with medical equipment and common safety practices.
- Strong documentation skills and ability to work collaboratively with medical teams.
- Excellent communication and crisis management skills.
- Current automobile insurance and reliable transportation for patient visits.
- Willingness to serve throughout the agency's geographic area as needed.
Essential Duties:
- Admit/readmit patients and provide comprehensive nursing services.
- Establish and maintain quality care standards in documentation and patient interactions.
- Document all patient services following state and federal regulations.
- Manage assignments efficiently, tracking patient progress and adjusting care as necessary.
- Develop written care plans and regularly supervise Home Health Aides.
- Continually enhance professional skills and maintain required certifications.
Ancillary Duties:
- Contribute to agency initiatives and support various health programs.
- Adhere to agency policies regarding personnel and administrative duties.
HIPAA and Privacy Compliance: Uphold patient confidentiality and adhere to agency policies regarding health information protection.
