At Merakey, we put our heart and soul into everything we do. We are seeking a Registered Nurse Case Manager to join our team at our program in Bucks, Montgomery, and Philadelphia counties. As a Registered Nurse at Merakey, you'll play a vital role in providing direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include clinical assessments, medication administration, and collaborating with a multidisciplinary team to ensure comprehensive care. Essential Job Functions: * Travel to group homes, deliver medications, update MARS and Treatment logs, Case Management, ensure physician orders are implemented, coordinate medical care, staff in-service on clients care plan, ensure medical charts and care are in compliance with regulations. Attend meetings, complete quarterly nursing reports and medical charting in data base. Assist in new admissions and/or readmission into group home. * Perform nursing assessments on residents, as needed, to identify and address health issues promptly. * Collaborate with various healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed. * Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager. * Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery. * Communicate health issues with the interdisciplinary team, actively participating in staff meetings. * Complete quarterly nursing reports. * Perform other duties as assigned, contributing to the smooth operation of our program. The ideal candidate will possess the following qualifications: * Graduation from an accredited Registered Nurse program. * Current license as an RN in the Commonwealth of PA. * Preferably, at least one year of experience as an RN and experience as a Case Manager and or Visiting Nurse preferred * Genuine interest in working with individuals with physical and developmental disabilities. * Possession of a current and valid PA driver's license, along with reliable transportation (may be required). * Ability to perform professional nursing duties including assessment, treatment, case management duties, computer literacy, experience with Outlook * Physical capability to exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.
