Supports ministry and system goals for capacity management through achieving length of stay goals, identification of avoidable delays at department approved levels, and other Case Management initiatives.nnCoordinates with the interprofessional team in meeting patient care goals.nnEstablishes partnerships with site Patient Experience champions to align efforts with patient satisfaction outcomes.nnIdentifies discharge needs, determines the appropriate level of care that the patient can transition to, coordinates planning, and problem solves continuity issues to ensure seamless continuum of patient care and prevent the patient from having to readmit to the hospital.nnEnsures all appropriate clinical information is provided to outside agencies.nnCommunicates pertinent information to the appropriate members of the patient care team both effectively and professionally.nnIdentifies nd eliminates barriers to effective patient stay management and promotes improvements in work processes within department, hospital, and community.nnPursues professional development opportunities and maintains licensure.nnAdvocates for patient legal, social, and ethical rights while balancing patient resources and hospital financial stewardship.nnRefers quality issues and poor utilization of services to the appropriate sources.nnCoordinates/facilitates patient/family education.nnEducates hospital leadership and colleagues to current case management practices, patient coordination issues, and unit specific trends.nnProvides consultation to professional colleagues and employees regarding patient stay options, financial considerations, psychosocial concerns, risk management, and ethical issues.nnDocuments pertinent assessments, interventions, transitions, and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.nnUtilizes computer systems.nnMaintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior.nnMINIMUM QUALIFICATIONSnnValid license as an RN. (Word, Excel, Power point).nnPHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONSnnMust be able to travel.nnMust be able to adapt to frequently changing work priorities.nnMust be able to set and organize own work priorities and adapt said priorities as they change frequently.nnManual dexterity is needed to operate a keyboard. Hearing is needed for extensive telephone and in person communication.nnPay Range: $47.00 - $50.00. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
