Our KORT Physical Therapy team is looking for a licensed and experienced, compassionate and driven In Home Physical Therapist. This is a treating clinical position and travel will be required to different home care settings, between centers, doctor’s offices, and to our patient’s homes for therapy treatments. Schedule: Monday-Friday and one weekend a month

Compensation: starting at $68,000 annual base and up commensurate with experience.

Responsibilities
n
n - In-Home Staff Physical Therapist evaluates, plans, and administers physical therapy treatment for patients suffering from injuries, muscle, nerve, joint, and bone disorders or diagnoses to restore function, relieve pain, and prevent disability.
n - Travel is required to different home care settings, between clinics, doctor’s offices, and to patient’s homes.
n - Evaluates the home environment for rehab and make appropriate recommendations to increase the independence of the client.
n - Develop, prepare, and maintain individualized client care progress records with accuracy, timeliness, and according to agency policy and procedures.
n - Supervise physical therapy assistants by directing, participating in, and presenting continuing education programs; and assume administrative/supervisory responsibilities in the absence of the director.
n - Educate client/caregiver and health care teams in the phases of physical therapy in which they may assist the client, in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate.

Qualifications

Minimum Qualifications
n
n - Licensed physical therapist in the state of employment.
n - Outpatient clinical rotation if a new grad
n - Outpatient experience in the last 5 years if a practicing clinician
n - Automobile liability insurance and current unrestricted driver’s license.
n - CPR certification or complete training required.

Preferred Qualifications:
n
n - Minimum one years of experience as a physical therapist in the outpatient setting
